| Requirements for Marriage Certificate |

Completed Marriage Certificate application form, |
|
Two passport-sized photos (with name written at the back), |
|
Your and your spouses' original passports, and |
|
Marriage Certificate processing fee is $25. |

Presence of two witnesses with two passport sized photos and Passport copies |

| All fees must be paid at the Bank after obtaining a deposit slip from the Embassy |

| Inheritance Form and Marrige Certificate form are available in the Embassy |

| Requirements for Power of Attorney |

Completed Power of Attorney form, (available in the Embassy) |

Two passport-sized photos (with name written at the back), |

Presence of two witnesses with their passports and two photos each |

Power of Attorney document processing fee: $100.00 |
