Visa Information
 
Welcome to the Visa Information page. Please complete the form with the required information, and return it to the Embassy with all the necessary attachments.
 
To download the forms please follow the following:
 
bullet Click below to download the desired Form. Please note that it does require the Adobe Acrobat Reader
bullet Once downloaded, select File > Print.
bullet Choose your Printer under "Printer Name"
bullet The desired Form is 8.5 x 11 so be sure to use "letter-sized" paper.
 
General Requirements
 
bullet Visa application form for each person,
bullet Two passport-sized photos (with name written at the back)
bullet Valid passport (valid for at least 6 months),
bullet For official passports (Diplomatic, Service and Student) a letter of
introduction from the employer or sponsored institution stating the
purpose and duration of the trip, and
bullet Visa processing fee of $30 for one month. Visa processing fee for visas over one month up to 6 months is equivalent to the increment of one month (i.e., 6 months visa is $180).

All fees must be paid at the Bank after obtaining a deposit slip from the Embassy.
